Visit Reason
The Minnesota Department of Health conducted a full survey at Vista Prairie At Goldfinch Estates on December 19, 2024, to evaluate and assess compliance with state licensing statutes.
Findings
The licensee was found in substantial compliance but had several deficiencies including food service violations, infection control issues, fire safety code violations, medication management problems, and oxygen storage concerns.
