If you’re lucky enough to be reading this, move on to the next community. We moved our mother in October. We could not get her out of there fast enough. November 13, we got her out. Go to https://nvdpbh.aithent.com/Protected/INS/SODPublicView.aspx?LicenseeId=114129&Program=HHF&CredentialType=AGC&LicenseeType=B&mode=&EntityType=LSE&LicenseNumber=10746&AddressTypeCode=PHL&LicenseId=137935&LikePopUp=&Mode=V&IsPopUp=Y There is a reason they have a D Grade for the past year. The managers and staff are not on the same page. They can change the name all they want, but it’s a honorably run care facility. Our mother was injured and it was not reported to us.
