Multiple deficiencies were cited including unclean and damaged ceilings, presence of unpleasant odors, facility hazards such as obstructed egress and unsecured oxygen bottles, and fire safety equipment not maintained in operating condition with obstructions and damaged fire-rated ceilings.

Biennial Construction Survey to assess compliance with physical plant, fire safety, and building code requirements for Walnut Ridge Assisted Living.

Findings
The facility failed to meet several physical plant and fire safety requirements including improper installation of emergency release switches on magnetic locks, inadequate fire separations and storage conditions, obstructions in corridors, unsafe housekeeping practices, non-functional fire safety equipment, compromised fire rated walls and doors, improper handling of medical oxygen cylinders, presence of prohibited portable electric heaters, and non-functioning exhaust ventilation in the Special Care Unit.

Storage room (Room B21) used for combustible storage without required sprinkler protection and fire barriers.
