Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for The Wealshire of Bloomington assisted living facility with dementia care license.
Findings
The licensee was found in substantial compliance but had several level two violations including failure to ensure food was prepared and served according to Minnesota Food Code, lack of a complete written emergency preparedness plan, failure to comply with State Fire Code including missing carbon monoxide alarms and controlled egress issues, incomplete fire safety and evacuation plans, failure to provide required designated representative notice in resident contracts, and failure to conduct hazard vulnerability mitigation. A $500 fine was imposed for fire protection and physical environment violations.
