Visit Reason
The Minnesota Department of Health conducted an initial survey on June 25, 2024, to assess compliance with state licensing statutes for Well Wish Residential LLC's assisted living facility license.
Findings
The licensee was found in substantial compliance but had several deficiencies including failure to list the licensed assisted living director as Director of Record, failure to prepare and serve food according to Minnesota Food Code and USDA guidelines, incomplete fire safety and evacuation plans, failure to provide required notice of right to designate a representative, lack of a complete treatment or therapy management plan, and failure to post required electronic monitoring signage.
