Visit Reason
The inspection was an unannounced routine inspection conducted to assess compliance with assisted living facility regulations and licensing requirements.
Findings
The inspection identified 15 rule noncompliances including deficiencies in employee training, emergency and disaster preparedness, personnel records maintenance, resident rights notification, housekeeping safety, and emergency drill documentation.

| The licensee did not ensure personnel records included termination date, reason for leaving, health inventory, TB skin test documentation, and documentation of core competency training. |
| The licensee did not ensure each resident received all their legal rights upon admission. |
| The licensee did not ensure a 30 day transfer notice was served upon one resident and the resident's responsible person when transferred. |
| The licensee did not ensure the notice of transfer was in writing and a copy was placed in the resident file. |
| Cleaning agents, dangerous and/or flammable materials were not locked to prevent unauthorized access. |
| Housekeeping personnel did not receive all required training. |
| The emergency and disaster response plan was not developed with state and local emergency disaster authorities and did not outline when to notify the Silver Alert program. |
| The emergency and disaster response plan did not have plans in place for severe weather, missing residents, death of a resident, interruption of public utilities, explosion, bomb threat, earthquake, windstorm, epidemic, or mass casualty. |
| The emergency and disaster response plan did not include assignment of personnel to specific tasks during an emergency; instructions on how to recruit additional help, supplies, and equipment to meet residents' needs after an emergency or disaster; delivery of essential care and services to facility occupants by alternate means; delivery of essential care and services if additional persons are housed in the facility during an emergency; and delivery of essential care and services to facility occupants if personnel are reduced by an emergency. |
