The inspection was conducted in response to complaint #189610 regarding trouble with the fire alarm panel at White River Assisted Living.
Complaint Details
Complaint #189610 regarding fire alarm trouble status. The complaint was investigated by CDSFM Christian and DSFM Harlan. The facility started fire watch but did not follow the 15-minute interval policy. The fire alarm panel network was still in trouble status at the time of inspection.
Findings
The fire alarm panel went into trouble status on 08/01/25 and was corrected on 08/05/25, but the communication radio network was causing issues notifying the facility. The facility's fire watch policy was not followed correctly, with 30-minute intervals instead of the required 15-minute intervals. The fire alarm panel network remained in trouble status at the time of inspection.

Visit Reason
An unannounced Fire and Life Safety Code re-inspection was conducted at White River Assisted Living by a representative of the Washington State Patrol, State Fire Marshal's Office to determine compliance with all applicable codes.
Findings
Multiple deficiencies were cited during the re-inspection, including issues with fire door latches, missing documentation for sprinkler system maintenance, and other fire safety violations. Many deficiencies from previous inspections were noted as corrected.

The inspection was conducted to investigate a complaint regarding the sprinkler system at White River Assisted Living.
Complaint Details
Complaint #176215 regarding the sprinkler system. The complaint was investigated and found no violations. No evacuation or injuries occurred. Fire Department responded due to fire alarm panel activation.
Findings
The elevator technician mistakenly cut the water line to the east portion of the community, causing water to be shut off for about 4 hours. No residents were harmed or evacuated, no fire occurred, and no IFC violations were observed.

On 07/09/2024 an unannounced Fire and Life Safety Code re-inspection was conducted at Living Court Assisted Living Community by a representative of the Washington State Patrol, State Fire Marshal's Office to determine compliance with all applicable codes.
Findings
The inspection found deficiencies related to door operation, duct and air transfer openings, and testing and maintenance of sprinkler systems. Some violations were corrected, while others required further action or documentation.

Swinging fire doors shall close from the full-open position and latch automatically.
The facility's fire damper report shows that 1 damper failed. The facility was unable to provide documentation showing it was corrected. The facility has all of their dampers inspected due to not knowing which damper failed. Four of the dampers failed and the contractor is working on correcting them.
The facility was unable to provide documentation for quarterly sprinkler reports and forward flow test; the facility indicated they are wanting to switch contractors and have not scheduled these tests yet.
