A separate, unannounced pre‑licensing inspection on 8 April 2025, attended by Licensing Program Analysts Sarah Benson and Kayla Adkison, focused on the facility’s readiness for a new management company. During the two‑hour visit the analysts noted that the center houses 49 residents—14 of whom are receiving hospice services—and is licensed for 56 non‑ambulatory residents. Observations revealed the facility in good repair, with functional dining and common areas, adequate hot‑water temperature, compliant food storage, and properly furnished bedrooms and bathrooms. Fire safety measures were confirmed, including inspection‑approved extinguishers and working exit alarms. The analysts also confirmed that knives are locked in the kitchen, the laundry area is secured, and all resident and staff files contain required paperwork. No deficiencies were identified against Title 22 Division 6 standards, and the analysts deemed the center ready to receive a new hospice waiver.

Across both visits, a pattern of consistent compliance emerges. Willow Springs demonstrates adherence to licensing mandates, effective staff training, and a safe physical environment, even amid a change in management. The reports highlight the facility’s proactive approach to regulatory readiness, particularly its attention to fire safety, hygiene, and resident documentation. No significant compliance issues or immediate risks were recorded, and the pre‑licensing inspection affirmed the facility’s suitability for continued operation under the new management company.
