Visit Reason
The Minnesota Department of Health conducted a survey to evaluate and assess compliance with state licensing statutes for Woodcrest Assisted Living.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including requiring residents to pay for meals as part of their contract, failure to maintain a tuberculosis prevention program with annual risk assessment, incomplete fire safety and evacuation plans and training, failure to label time-sensitive medications with open and expiration dates, and unsafe storage of chemicals accessible to residents.
