The inspection was conducted in response to a complaint received by VDSS Division of Licensing on January 8, 2025, regarding allegations in the areas of Staffing and Supervision, Personnel, Building and Grounds, Resident Care and Related Services, Resident Accommodations and Related Provisions, and Administration and Administrative Services.
Findings
The investigation supported some, but not all, of the allegations. Areas of non-compliance were found in Resident Accommodations and Related Provisions, Resident Care and Related Services, and Personnel. Several violations were cited including lack of staff certification in first aid and CPR, inadequate personal care for residents, unsecured medication storage, improper medication administration documentation, and infrequent changing of bed linens.
