The California Department of Social Services Community Care Licensing Division conducted a pre‑licensing visit to SAFE HAVEN MANTECA LLC on July 30 2025. Licensing Program Analysts Charlie Yang and Noel Peterson met with facility representatives Caitlin Robinson and Aileen Grimesey and received an interview that clarified the facility’s intent to accommodate up to six non‑ambulatory residents, including a hospice waiver for four residents and a pending dementia care program. At the time of the inspection the census was zero, but the evaluators toured all resident‑ready areas, noting that furniture, bedding, and restrooms met state requirements, that grab bars and non‑skid mats were in good repair, and that hot water temperatures were within the permitted range.
Key safety and compliance checks were completed. All kitchen drawers containing knives and sharps were locked, and cleaning agents, bleach, and other hazardous supplies were secured and inaccessible to residents. The medication cabinet, located in the living‑room hallway closet, was also locked; the medication administration record and dispensing log were reviewed and found to be in order. Fire extinguishers had been inspected on March 13 2025 and were compliant, and the administrator’s certification (no. 6068452740) was current through December 2 2025. No deficiencies were documented, and the report indicates the facility is in compliance with applicable regulations at the time of the visit.
The evaluators recorded that all documentation, including the facility’s resident and personnel files, met licensing standards. The facility’s garage area was not used by residents, and the perimeter fence and exit points were inspected. The report, available for public review for three years, did not list any civil penalties or corrective actions, confirming that SAFE HAVEN MANTECA LLC met all required safety, health, and administrative standards during this pre‑licensing inspection.
