SAFE HAVEN OAKLEY LLC, a 6‑bed residential care facility located at 228 Golden State Parkway in Oakley, completed a series of licensing reviews that trace the facility’s progress from initial registration to full licensure. The first report, dated September 13 2024, documented a brief office visit in which Administrator Ryan Q. Alejo verified his identity, confirmed understanding of California’s Community Care Facility licensing laws, and signed the required LIC 809 form. No deficiencies were identified; the visit confirmed compliance with Component II and prepared the application for the next stage.
On October 2 2024 an announced pre‑licensing inspection was conducted by Licensing Program Analyst L. Hall. The examiner toured the entire facility—bedrooms, bathrooms, common areas, kitchen, and outdoor spaces—and found that all safety and environmental requirements were met. Fire clearance, hospice waiver, and essential equipment such as fire extinguishers, carbon monoxide and smoke detectors, and a complete first‑aid kit were all in compliance. The Licensing Program Manager waived Component III, and the report noted that the facility was ready for licensing pending final approval by the Central Applications Unit. At this point the census remained at zero, but the facility’s physical readiness was confirmed.

While the environment, temperature controls, and supplies continued to meet standards, several deficiencies were documented. Staff records lacked required postural support, restricted condition, and dementia care training, and three of the four residents did not have physician orders for bedrails. These findings, cited under Section 1569.696(a) and CCR 87608(a)(5)(A), were classified as Type B deficiencies and required corrective action by May 21 2025. The licensing analyst outlined a plan of correction that included staff training and submission of updated documentation to the California Department of Social Services.
