The September 2017 complaint investigation substantiated 3 deficiencies regarding failure to validate staff competency, failure to report abuse allegations to the Health Care Personnel Registry, and substantiated physical and verbal abuse of 5 residents by facility staff member who was terminated. The March 2017 annual survey identified 4 deficiencies in staff competency validation, resident supervision, and medication review protocols. Earlier construction surveys from 2015 to 2017 documented recurring fire safety and structural deficiencies.
