November 2025’s dementia care disclosure noted no current deficiencies.

Occupancy has declined from 30 in mid-2020 to 25 as of late 2025. No monetary penalties or license actions were issued.

The Chateau’s recent pattern of deficiencies, particularly in abuse reporting, staff training, background verification, and infection control, reflects ongoing operational challenges addressed through corrective action plans completed or targeted for completion by December 2025.
