Visit Reason
Unannounced routine inspection to assess compliance with assisted living facility regulations.
Findings
The inspection identified multiple deficiencies including lack of staff identification badges, missing quality assurance meetings, incomplete employee orientation and training, missing food handler permits, unlocked medication room, inadequate emergency disaster plan, and incomplete criminal background screening documentation.

R432-1-4: Staff do not have identification badges as required.
R432-270-6(2): No quality assurance meeting was held for the first quarter of 2024.
R432-270-9(7)(a)-(f): Orientation was not provided to one employee within 30 days of hire and the orientation form lacked employee name and signature.
R432-270-9(8)(a)-(c): Sixteen hours of documented one-on-one job training was not completed for one new employee.
R432-270-9(9)(a)-(l): No annual employee trainings were completed on four employees reviewed.
R432-270-10(2)(a)-(b): Resident rights notification did not include statement about filing complaints with the state long-term care ombudsman and other advocacy groups.
R432-270-22(8)(a)-(c): Two employees who handle food did not have current food handler's permits.
R432-270-23(5): Chemicals were stored in the kitchen and public restroom, including the shower area.
R432-270-26(2)(a)-(c): Emergency disaster plan was not coordinated with state and local emergency disaster authorities and lacked required elements including missing resident, interruption of public utilities, and epidemic.
R432-270-26(6)(a)-(j): Emergency and disaster response plan did not include names of persons in charge, notification priorities, instructions for recruiting additional help, and delivery of essential care under emergency conditions.
R432-270-26(8)(a)-(d): Licensee did not hold simulated fire drills for the first quarter of 2024 and did not include residents' ability to evacuate during fire and disaster drills in 2023.
R432-270-35-4(2)(a)-(b): No criminal background screening authorization form for three employees and no fingerprints in DACS for one employee.
R432-270-19(17)(a)-(b): Medication room was left unlocked and accessible.
