Visit Reason
The Department conducted a complaint survey investigation at Raintree Terrace from March 5, 2018 through March 7, 2018 following concerns about a resident's safety and elopement risk.
Complaint Details
The complaint investigation was substantiated based on findings that Resident #9 was at risk for elopement and the facility did not provide adequate safety measures to prevent the resident from leaving the building unnoticed.
Findings
The investigation found that Resident #9, diagnosed with Alzheimer's dementia and at risk for elopement, was able to leave the facility unnoticed due to a secured unit door that did not alarm and staff not hearing the courtyard gate alarm. The facility failed to provide adequate safety measures for the resident.

Rule 1200-08-25-.07(7)(a)(2) requires each ACLF to provide safety for residents. The facility failed to ensure safety when Resident #9 eloped due to a non-alarming secured unit door and staff not hearing the courtyard gate alarm.
