The California Department of Social Services conducted a series of unannounced investigations at AEGIS Living Ventura (Facility #565850141) over a twelve‑month period. Four distinct complaints were reviewed, each alleging deficiencies in supervision, resident safety, or communication with residents’ responsible persons. The allegations ranged from a resident eloping without supervision, to a fall that resulted in a fractured wrist, to the facility’s failure to reply to a responsible person’s correspondence, and concerns that staff were not meeting a resident’s needs.
In every instance, the investigators—evaluators Zabel Chochian, Kelly Dulek, Joann Rosales, and Martha Arroyo—conducted staff interviews, reviewed incident reports and medical records, and assessed staffing schedules. The findings consistently indicated that the documented incidents had been recorded appropriately, that staffing levels were adequate, and that no evidence of neglect or supervisory failure was found. For the fall that led to a wrist fracture, the records showed the resident had a history of falls, was on anticoagulation therapy, and that the fall was documented in the incident reports without signs of abuse or neglect. Likewise, the claim that a resident had eloped was unsupported by any record of an elopement event.
All four investigations concluded with determinations of Unfounded or Unsubstantiated, meaning the allegations lacked a reasonable basis or sufficient evidence to support a violation of licensing standards. The outcomes reaffirmed that the facility maintained compliance with state supervision and documentation requirements during the periods examined. While the repeated complaints point to resident concerns about supervision and communication, the state’s findings highlight that the facility’s processes, staffing, and incident reporting met the required standards, and no corrective actions were mandated.
