Inspection Report Summary for Alexandria Victoria II
The 2023 unannounced annual inspection of Alexandria Victoria 2, conducted by LPA Ryker Heberle, found the facility in full compliance. All staff and residents were vaccinated, the infection‑control plan had been submitted, and the environment—living areas, kitchen, dining room, bathrooms, and storage spaces—was clean and well‑maintained. Entry and exit routes were clear, visitor screening was in place, and emergency equipment such as fire extinguishers, smoke and carbon‑monoxide detectors, and first‑aid supplies were functional. No deficiencies were cited, and the Administrator, John Gryspos Jr., received a signed copy of the report.
The following year, a similar unannounced inspection by LPA Steve Chang revealed a series of safety and compliance deficiencies. A carbon‑monoxide detector was found out of battery, and both the dish‑washing detergent cabinet and the knives cabinet in the kitchen were unlocked, creating immediate health and safety risks. Bathrooms lacked non‑skid mats, and an obstruction in the left‑side yard impeded safe passage. Staffing issues emerged when two employees lacked current first‑aid certificates and one staff member’s health‑screening record was missing. The facility’s fire and emergency drill schedule was only up‑to‑date to August 2023, and one of the carbon‑monoxide detectors was out of battery. The Administrator promptly replaced the detector battery, installed locks on the hazardous storage cabinets, and placed non‑skid mats and cleared the yard before the LPA departed. Plans of correction were established with due dates ranging from 2 to 21 days, and the Administrator committed to submitting the required documentation.
Across both inspections, a clear pattern emerges: while the 2023 visit indicated robust infection control, visitor protocols, and general facility upkeep, the 2024 inspection exposed gaps in hazard mitigation, staff competency, and record‑keeping. The repeated emphasis on lockable storage for cleaning agents and weapons, functional environmental detectors, and up‑to‑date training records underscores the department’s focus on immediate health and safety risks. The Administrator’s timely remedial actions and scheduled plans of correction reflect an awareness of these regulatory priorities, although the persistence of several deficiencies highlights an ongoing need for diligent compliance oversight.
