Inspection Reports for Alice Lorraine Care Center

The visit was conducted as a renewal inspection to determine compliance with applicable licensing statutes and rules for the Alice Lorraine Care Center.

The facility was found to be in substantial compliance with all applicable rules and statutes. The licensing consultant recommended issuance of a 2-year regular adult foster care license.

The investigation was initiated due to a complaint alleging that staff member Shalynda Tinkey forcefully grabbed Resident A's arm, flung her up from the toilet, and used profane language towards her.

The investigation corroborated the allegation that Resident A was not treated with dignity and respect, and her personal needs, including protection and safety, were not attended to at all times. Staff member Ms. Tinkey was found to have acted inappropriately and subsequently quit. Resident A suffered a bruise consistent with the incident.

The complaint alleged that on 11/01/22, staff member Shalynda Tinkey forcefully grabbed Resident A's arm, flung her up from the toilet, and stated, "She is fucking lazy." The allegation was substantiated based on interviews, written statements, and observation of a bruise on Resident A.

The visit was conducted as a final inspection following approval from the Office of Fire Safety to review a physical space addition to the facility.

The facility added a physical space with eight new bedrooms, each with its own bathing facility, and additional outside patio space. The addition did not increase the licensed bed capacity, which remains at 20 beds.
