The inspection was conducted to assess compliance with regulatory requirements related to resident care, abuse reporting, medication storage, food safety, infection control, and other facility operations.
Findings
The facility was found deficient in multiple areas including failure to ensure staff dignity during feeding, delayed reporting of physical abuse, unsecured controlled medication storage, improper food temperature monitoring and labeling, use of dirty plates, and inadequate infection control practices related to laundry handling.

Failure to timely report an allegation of physical abuse to the State Survey Agency within the required two-hour timeframe.
