C. A manager shall ensure that a personnel record for each employee or volunteer: includes required personal and employment information; failed to establish and maintain a personnel record for employee E5.
C. A manager shall ensure that a caregiver or an assistant caregiver documents the services provided in the resident's medical record; failed to document services for two residents.
B. If an assisted living facility provides medication administration, a manager shall ensure that a medication administered to a resident is documented in the resident's medical record; failed to document medication administration for two residents.
F. When medication is stored by an assisted living facility, a manager shall ensure that medication is stored in a separate locked room, closet, cabinet, or self-contained unit; medications were stored unlocked in the kitchen refrigerator.
