Inspection Report Summary for Bellara Senior Living

Bellara Senior Living (Facility No. 019201373) is a 740‑type senior‑living home in Hayward, California, with a licensed capacity of 175 residents. The Department’s recent visits—two unannounced compliance trainings in September and October 2024 followed by a complaint investigation in June 2025—have consistently shown that the facility is meeting its licensing obligations and that no significant violations have been identified.

In late September, a Component II compliance training was conducted by the Centralized App Unit. The administrator and director, Jeff Sumabat, and executive director Ryan Leong participated via a telephone call. The review covered Title 22 operations, staff qualifications, resident policies, and program documentation. The LPA confirmed that the facility’s personnel understand and follow the required policies and that all necessary certifications—such as administrator and staff health clearances—are up to date. A second unannounced visit in mid‑October carried out a Component III training, again attended by Jeff Sumabat and regional operations vice president Zach Butcher. The training, delivered by LPA Jill Clancy‑Czuleger, reinforced program policies and addressed any questions from the facility’s leadership. Both visits ended with signed acknowledgments from the facility representative, indicating receipt of the training and understanding of their appeal rights.

The most recent inspection, in June 2025, was prompted by a complaint alleging inadequate food service for residents. Licensing Program Analyst James Sampair entered the facility unannounced, interviewed the executive director and nine residents, and collected witness statements. The evidence gathered demonstrated that the staff were indeed providing adequate meals, and no preponderance of evidence supported the complaint. Consequently, the allegation was deemed unsubstantiated, and no enforcement action was taken.

Across these three events, Bellara Senior Living has displayed a pattern of proactive compliance: it repeatedly accepts unannounced trainings, involves its leadership in review processes, and promptly addresses concerns. The lack of substantiated findings suggests the facility maintains satisfactory standards of care and safety, while the repeated engagements with the licensing office reflect a collaborative relationship focused on continuous improvement.
