Visit Reason
The inspection was conducted to assess compliance with regulatory requirements related to resident dignity, accurate resident assessments, food service quality, food safety, and infection prevention and control at Bibb Medical Center Nursing Home.

Findings
The facility was found deficient in multiple areas including serving meals on Styrofoam trays instead of plates affecting resident dignity, inaccurate coding of resident assessments, serving cold and unpalatable food, improper food storage and temperature monitoring, and failures in infection prevention practices such as inadequate hand hygiene and improper handling of clean linen.

Deficiencies (5)
Failure to ensure Resident Identifier #38's lunch meal was not served on Styrofoam on 11/27/2023 and staff delivering laundry entered residents' rooms without permission.
Failure to ensure Resident Identifier #64's annual MDS assessment was accurately coded for weight loss or gain and Resident Identifier #114's quarterly MDS assessment was accurately coded to reflect no anticoagulant medication use.
Failure to ensure food was served warm, palatable, and enjoyable; food temperatures on test trays were below safe levels.
Failure to ensure food stored in the freezer was covered and sealed properly, outdated food was discarded, and temperature of blended foods was measured before serving.
Failure to ensure staff performed hand hygiene when delivering meal trays, handling clean linen, and distributing resident clothing; failure to cover clean linen during transport; and improper handling of clean linen against personal clothing.

The inspection was conducted to assess compliance with regulatory requirements related to continuing education for nursing assistants, accuracy of meal tray service according to resident preferences, and adherence to prescribed therapeutic diets.

Findings
The facility failed to ensure 27 of 29 CNAs completed the required 12 hours of continuing education annually. Additionally, the facility did not consistently honor residents' specified food preferences for at least two residents and failed to follow salt or sodium restricted diets for two of three residents observed.

Visit Reason
The inspection was conducted to evaluate compliance with food safety and hygiene standards in the nursing home's dietary services, including food temperature monitoring, cleanliness of food carts, and handwashing practices of dietary staff.

Findings
The facility failed to ensure that food temperatures were consistently taken on the tray line, food carts used to transport resident meals were clean, and staff washed their hands when entering the kitchen. These deficiencies had the potential to affect all 113 residents receiving meals.
