Routine inspection of Brandenburg Nursing and Rehabilitation Center to assess compliance with regulations regarding resident safety, environment, care plans, medication storage, and facility maintenance.
Findings
The facility failed to provide a safe, clean, comfortable, and homelike environment due to maintenance issues such as broken bathroom fans, damaged vanities, loose handrails, and exposed sharp edges on radiators. Care plans for fall risk were not consistently followed. Medication carts and medication refrigerator were not securely locked, posing safety risks. Several environmental hazards were identified including loose handrails, exposed wood, and unsecured radiators.
