Visit Reason
The Minnesota Department of Health conducted a follow-up survey on October 13, 2023, to determine correction of orders found on the survey completed on August 16, 2023.
Findings
The follow-up survey determined the facility had not corrected all of the state correction orders issued pursuant to the August 16, 2023 survey. The licensee is in substantial compliance but must take action to correct the state correction orders and document the actions taken.
