Inspection Report Summary for Brookhaven Health Care Facility

During the reporting period, the facility received a total of 20 citations, comprising 17 standard health citations and 3 Life Safety Code citations. These citations were issued across six inspections that resulted in citations, while seven additional inspections concluded with no citations. Of the 20 citations, one involved actual harm to a resident, representing 5.0 % of all citations.
