This place, d.b.a. Autumn Lake Healthcare of Greenfield, is a disaster and should be closed. My spouse was there from 2/13/25–2/15/25. During the course of her stay, she was not given ordered medications, not attended to when called and needed, and verbally abused by a staff person who called both her and me 'assholes'! A complaint about the facility was lodged with the State of WI Dept. of Health. The investigation resulted in a 58-page report and the issuance of 10 federal citations. The most serious citations are F609, F610, F656, F686, F689, F692, F760, F773, and F880, cited at a severity/scope level of D (Potential for Harm/Isolated).
