The inspection was conducted as part of the facility's annual survey to assess compliance with regulatory requirements related to resident care, medication management, hospice services, and assistive devices.

Findings
The facility was found deficient in multiple areas including failure to ensure proper use and management of hearing aides for a resident, inadequate monitoring and care planning for a resident on anticoagulant therapy, lack of targeted behavior monitoring for a resident on psychotropic medication, and failure to coordinate hospice care documentation and communication for a resident receiving hospice services.
