Inspection Report Summary for Clove Lakes Healthcare and Rehabilitation Center

During the reporting period, the facility received a total of 36 citations, comprising 24 standard health citations and 12 Life Safety Code citations. These citations were identified during five on‑site inspections that resulted in citations, while seven inspections concluded with no citations. The citation summary indicates that none of the citations involved actual harm or immediate jeopardy to residents. The total number of citations of any kind received by the nursing home during the reporting period was 36, compared with a state average of 31.

The November 10, 2022 certification/complaint survey produced the largest number of cited items, with ten standard health deficiencies and ten Life Safety Code deficiencies. Key deficiency areas identified in that survey included care plan timing and revision, development and implementation of comprehensive care plans, drug regimen review, food procurement and preparation, elimination of accident hazards, infection prevention and control, physician visit adequacy, reporting of alleged violations, respiratory/tracheostomy care and suctioning, and the right to be free from physical restraints. Life safety deficiencies involved electrical system integrity, emergency lighting, gas equipment storage, hazardous area enclosure, means of egress, multiple occupancy arrangements, smoking regulations, sprinkler system maintenance and testing, subdivision of building spaces for smoke comparison, and vertical opening enclosure. This inspection represented the most extensive set of deficiencies recorded for the facility.

Complaint metrics for the period from August 1, 2021 to July 31, 2025 show that the facility received 170 complaints, equating to 33.7 complaints per 100 occupied beds. Forty‑nine percent of complaints represented facility‑reported incidents, and 39.0 % of complaints were facility‑reported incidents.
