Visit Reason
The inspection was an unannounced routine inspection conducted to assess compliance with assisted living facility regulations.
Findings
The inspection identified 12 rule noncompliances related to various regulatory requirements including administrator duties, resident assessments, service plans, employee training, food services, housekeeping, emergency preparedness, and pet vaccination records.

| One resident assessment did not include a statement signed by the healthcare professional completing the assessment that the resident met criteria for the facility and was not accurate at the time of assessment. |
| One resident service plan did not include who would provide the services. |
| 3 terminated employee files were not complete. |
| No quarterly dietician consultation for individuals with therapeutic diet orders. |
| One employee who serves food did not have a food handler's permit. |
| Disinfectant, rubbing alcohol, disinfectant wipes, and stain remover were found in unsecured locations. |
| The facility did not have current vaccination records for pets. |
| The licensee did not document the resident's ability to evacuate on all fire drill documentation and did not hold a simulated fire drill during the overnight shift for the 3rd quarter of 2023. |
