A pre‑licensing visit on 9 / 16 / 2021 found the property fully equipped—six bedrooms, three bathrooms, a functional kitchen and dining area, adequate fire and safety equipment, and an adequate food supply—yielding no citations and a recommendation for licensure.

The first unannounced, required one‑year inspection in September 2022 reiterated that the facility remained compliant with all physical plant, health, and safety regulations. Fire safety equipment, emergency lighting, and lockable storage for chemicals, knives, and medications were in place, and the staff was fully vaccinated and trained on infection control. The LPA noted no staffing or policy deficiencies and issued no citations, confirming that Gem Oaks met Title 22 standards in 2022.

A second unannounced inspection on 9 / 13 / 2023 highlighted a lapse in food safety and medication storage. The LPA discovered expired produce, dairy, and cheese in the refrigerator and noted that refrigerated medications were left unlocked. While the facility promptly removed expired items, the incident underscored a need for stricter inventory controls and lockbox use for temperature‑sensitive drugs. No other violations were noted, and the overall safety and compliance posture of the facility remained sound.

Across the three inspections, Gem Oaks consistently met regulatory standards for physical plant, infection control, and staff qualifications. The 2023 findings point to a single, addressable lapse in food and medication management, suggesting that ongoing oversight and staff training on inventory procedures will maintain the facility’s high level of compliance.
