McClurg, met with Administrator Zainab Younes and conducted a comprehensive, unannounced inspection of all interior and exterior areas, including the dining room, common spaces, kitchen, resident rooms, bathrooms, and the surrounding yard.
The audit documented that all safety and health standards were satisfied. Fire clearance for all non‑ambulatory residents was confirmed, and a hospice waiver for five residents was in place. Resident rooms and bathrooms were well furnished, lit, and equipped with grab bars, non‑slip mats, and adequately hot water (113–115 °F). Hazardous cleaning agents and medications were stored in locked areas inaccessible to residents, and the facility’s signal system and personal protective equipment supplies were adequate. No odors, debris, or other sanitary concerns were noted, and all passageways were clear and unobstructed.
The inspector’s narrative concluded that the facility met all applicable licensing requirements and “has no deficiencies.” The pre‑licensing evaluation was therefore considered complete, and the facility was cleared for licensing without civil penalties.
