GOLDEN GRACE DAY ACTIVITY & HEALTH CARE SERVICES LLC underwent a comprehensive inspection on October 18, 2021, resulting in 18 state standard violations. Of these, 17 were classified as Life Safety Code deficiencies, with the remaining one categorized under Health Code. The Life Safety Code issues included fire safety and building maintenance concerns, such as inadequate fire alarm systems, insufficient sprinkler system inspections, and subpar lighting levels. Additionally, there were problems with resident care, including a lack of documentation for staff competency and training, and an incomplete COVID-19 response plan. Some of these deficiencies had been corrected by November 5, 2021. No enforcement actions were found in relation to this inspection.
