Golden Living Health Management, Inc. (facility number 374602369) received a series of regulatory visits between September 2021 and May 2022. Early in the period, two case‑management visits—one prompted by a self‑reported incident of alleged staff abuse (September 2021) and a second licensee‑initiated meeting (November 2021)—concluded without any deficiencies. The facility was satisfied with the resident care and staff conduct as presented during those encounters. A technical‑assistance visit in January 2022, focused on the facility’s COVID‑19 mitigation plan, also yielded no citations and reinforced compliance with infection‑control protocols.

The third visit, an unannounced annual required inspection in March 2022, again found the facility in compliance with state statutes and regulations, particularly with respect to dementia care, incidental medical services, and resident rights. The inspection emphasized the importance of ongoing adherence to CCR 87705, 87465, 87468, 87463, and 87211, but no violations were documented.

Turning to safety and documentation, the situation shifted in late April and early May. On April 29, 2022, a case‑management incident visit uncovered that Resident #1 had left the facility unassisted and was hospitalized. The review revealed a missing current resident appraisal and a deficiency was issued under CCR 87467, highlighting the facility’s failure to maintain timely assessments and to follow elopement protocols. A subsequent unannounced visit on May 18, 2022, addressed deficiencies identified during a complaint investigation. It was found that Resident #1’s physician report was not updated within the required 12‑month period and the report prohibited unassisted departure, yet the resident was still allowed to leave unassisted. The report also noted that the facility had not implemented elopement protocols for residents with such restrictions. These findings prompted additional deficiency citations, underscoring gaps in resident record accuracy and policy enforcement.

Overall, Golden Living Health Management demonstrated a pattern of compliance with most regulatory expectations, but late‑2021 through mid‑2022 revealed significant lapses in resident documentation and elopement safety. The deficiencies highlighted the critical need for rigorous, up‑to‑date resident assessments and strict adherence to discharge restrictions to safeguard resident welfare.
