Inspection Report Summary for Greene Meadows Nursing

During the reporting period, comprising 12 standard health citations and 2 life safety code citations. These citations resulted from two inspections that identified deficiencies; an additional 11 inspections were conducted and yielded no citations. The total number of citations of any kind received by the center during the reporting period was 14, compared with a state average of 31.

The inspection conducted on September 14, 2023 identified the largest number of cited items, with 13 citations recorded. Deficiencies were concentrated in standard health areas, including the development and implementation of comprehensive care plans, improper garbage disposal, inadequate food procurement and preparation, lack of accident hazard controls, unresolved grievances, substandard nutrition and personal food policy, deficiencies in quality of care, violations of resident rights, and an overall unsafe environment. Life safety deficiencies noted during the same inspection involved elevator performance and improper storage of gas cylinders and containers.

During the reporting period, the center received 88 complaints, equating to 81.2 complaints per 100 occupied beds. Twenty‑four percent of complaints represented facility‑reported incidents. Eleven complaint‑related on‑site inspections were completed. The center incurred nine complaint‑related citations, with one citation falling under the resident rights category; all other complaint‑related citations were zero. The rate of complaint‑related citations per 100 occupied beds was 8.3, compared with a statewide rate of 4.1.

Enforcement actions taken against the center include five stipulation and order determinations. NH‑17‑036, issued May 22, 2017, imposed a $10,000 fine for quality of care deficiencies. NH‑16‑055, issued January 21, 2016, imposed an $18,000 fine for multiple deficiencies. NH‑16‑099, issued January 13, 2016, imposed a $10,000 fine for quality of care.
