Visit Reason
The inspection was conducted as an End of Provisional Survey to assess compliance with the Regulations Governing the Licensing and Functioning of Assisted Housing Programs: Level I Residential Care Facilities.

Findings
The facility was found not in substantial compliance due to multiple maintenance deficiencies including damaged drywall, missing wall tiles and wallpaper, plaster missing from the bathroom ceiling, rotting exterior siding and window frames, debris accumulation on the roof, and failure to maintain resident areas at a minimum temperature of 70°F.

Deficiencies (7)
Interior hallway leading to resident bedrooms had gashes in the drywall along the lower area of the wall.
Wall tiles and wallpaper were absent/torn from the wall in the kitchen near the refrigerator.
Two sections of plaster were missing from the bathroom ceiling.
Exterior siding of the house appeared to be rotting from water damage with exposed wood behind the siding and dark blemishes on the front siding.
An exterior window frame was broken with exposed and damaged wood at the rear of the house.
An approximately nine-foot wide section of the lower half of the roof at the rear of the house was covered with leaves, moss, branches, and two metal poles causing debris build-up.
Resident areas of the facility failed to maintain a temperature of at least seventy degrees Fahrenheit; living room heater was set to 58°F and room temperature was 61°F.
