Inspection Reports for Heatherwood Memory Care

The inspection was an unannounced Required Annual Inspection conducted to evaluate compliance with licensing requirements at the Heatherwood Memory Care Facility.

The Licensing Program Analyst toured the facility and inspected various areas including kitchen, storage, dining, restrooms, living spaces, and client rooms. All safety equipment was operational, food storage met requirements, and required postings were present. No citations or deficiencies were issued during the inspection.

An unannounced complaint investigation visit was conducted following a complaint received on 2024-10-21 regarding an alleged risk posed by a non-client individual residing or present at the facility.

The investigation found that the alleged individual was not present in or associated with the facility, and the complaint was determined to be unfounded.

The complaint alleged that a non-client (RSO) resided, was present, or had contact that may pose a risk to the health and safety of clients. The complaint was investigated and found to be unfounded.

The inspection was an unannounced 1-Year Annual Required inspection conducted to assess compliance with licensing requirements.

The facility was toured and various safety and care aspects were reviewed, including resident apartments, common areas, and records. No deficiencies were cited during the visit.

Unannounced complaint investigation visit conducted due to a complaint alleging that the licensee was not maintaining liability insurance.

The investigation found that the licensee provided a valid Certificate of Liability Insurance with an effective date of 08/01/2023. The allegation was closed as unfounded with no deficiencies cited.

Prelicensing inspection conducted due to Change of Ownership to evaluate readiness for licensing.

No issues were noted during the inspection. The facility was found ready to be licensed, with proper furniture, safety equipment, and environmental conditions observed.

The visit was a Case Management - Other type, involving a face-to-face Component III presentation and discussion of regulations with the Assistant Administrator.

Licensing Program Analysts conducted a presentation on Component III regulations, observed that the participant gained knowledge about running and maintaining the facility in accordance with regulations, and provided a copy of the report during the exit interview.

The visit was conducted as part of a Change of Ownership application process for the Heatherwood Memory Care Facility.

The applicant and administrator participated in a COMP II telephone interview to verify identification and confirm understanding of California Code Title 22 regulations, including facility operation, admission policies, staffing, health conditions, emergency preparedness, complaints, and pre-licensing readiness.
