Inspection Report Summary for Ivy Park at Escondido
On April 29, 2025 the California Department of Social Services Community Care Licensing Division conducted an unannounced pre‑licensing inspection of Ivy Park at Escondido, a residential care facility for the elderly with a 123‑resident capacity. The Licensing Program Analyst, Janira Arreola, met with Administrator Samuel De Guzman, reviewed the facility’s records, interviewed staff, and walked through the building. The inspection confirmed that the property, a two‑story structure with resident activity rooms, bedrooms, bathrooms, staff offices, laundry, and storage spaces, is clean, sanitary, and fully equipped with required furniture and safety devices. Fire inspection results show approval for 116 non‑ambulatory and seven bedridden residents in specified rooms, and the facility’s smoke and carbon‑monoxide alarms were functioning.
Additional observations indicated that hygiene, PPE, and cleaning supplies are properly stored and locked away from residents, while the kitchen meets department standards for food preparation and hot‑water temperature. Outdoor areas were free of hazards and included a shaded space and emergency exit. Records review revealed complete resident documentation, an accurate staff roster with all criminal record clearances, and that the facility’s first‑aid kits, emergency water, food, and lighting supplies are in place. The landline telephone at (760) 747‑4888 was operational, and the activity calendar and menus were posted for residents.
No deficiencies were identified during the visit, and therefore no corrective action or civil penalty was required. The report states that there are no objections to proceed with the pre‑licensing process, allowing the facility to continue its application for a change in ownership. The inspection report was signed by the licensing evaluator and the facility representative, confirming receipt and understanding of licensing rights. The document will remain publicly available for three years as required by California law.
