All furnishings, safety equipment, and environmental conditions met state standards, and staff and resident files were complete. The visit concluded with no deficiencies and the report was forwarded to the Central Applications Unit for final review.
Later that afternoon, a Component III case‑management session with Executive Director Gilbert Castro confirmed that the facility remained compliant with all regulatory requirements. The analyst presented a power‑point summary of the component and provided a copy of the report to the director. No additional concerns were identified.
On 16 October 2024, a separate, unannounced investigation was triggered by Complaint No. 15‑AS‑20240520111558. LPAs Ardalan Gharachorloo and Lori Alexander found that the facility could not provide May 2024 shower or toileting schedules, staff rosters, or the complete resident file for R1. These record‑keeping lapses constitute a Type B deficiency under 22 Cal. Code Reg. § 87506. A Plan of Correction was issued with a due date of 25 October 2024, warning that failure to correct could result in civil penalties.
A follow‑up case‑management visit on 25 April 2025 reiterated the missing documentation issue. LPAs observed that R1’s file was stored off‑site, and the Business Office Director confirmed that records had been relocated to storage outside the facility. The deficiency, again cited under the same regulatory section, was noted with the same potential for civil penalties if not remedied. This pattern highlights an ongoing challenge with resident‑record accessibility, despite initial compliance and routine case‑management oversight.
