The most recent inspection on July 24, 2025, found no deficiencies during the recertification visit for the Assisted Living Program. Earlier inspections showed a generally compliant record with isolated deficiencies related to medication administration certification, incident reporting, and staff record checks. Inspectors cited issues mainly with medication manager certification and incomplete incident reports following accidents, as well as one instance of missing required background checks for a staff member. Complaint investigations were mostly unsubstantiated except for a substantiated complaint in 2021 regarding uncertified medication administration and a 2018 complaint about missing incident reports. The facility’s recent inspections indicate improvement, with the last two recertifications showing no deficiencies.
