An unannounced visit was made to Kindcare Assisted Living Of Bristol on July 21, 2025 by a representative of the Facility Licensing and Investigations Section of the Department of Public Health for the purpose of conducting a relicensure.
Findings
The Governing Authority failed to implement Public Act No. 21-185, Section 7, by not ensuring the establishment of a family council in the dementia special care unit and staff were unaware of the Public Act requirements.

An unannounced visit was made to Kindcare Assisted Living Of Bristol on December 22, 2023, by a representative of the Facility Licensing and Investigations Section of the Department of Public Health for the purpose of conducting an investigation.
Findings
The agency failed to ensure a client's safety and failed to immediately report an abuse allegation to agency administrative personnel. The allegation of abuse was investigated and found to be unsubstantiated. The agency's abuse policy requires immediate reporting and protective measures for clients.
