Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for Legend Health Care Resources.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including failure to document actions taken to comply with previous correction orders, failure to ensure food service compliance with Minnesota Food Code, lack of tuberculosis prevention program, non-functioning interconnected smoke alarms, maintenance issues, incomplete dementia and mental health training for staff, and failure to conduct timely resident reassessments.
