Complaint Details
The complaint investigation was substantiated based on the findings related to Resident #1's elopement and the facility's failure to maintain awareness of the resident's whereabouts.
Findings
Resident #1 eloped from the facility due to lack of preventative measures despite a known history of wandering. Staff failed to recognize the egress alarm, resulting in Resident #1 being missing for approximately five hours and sustaining skin tears and abrasions.

The complaint involved Resident #1 exhibiting aggressive behavior including yelling, attempting to elope, and physically assaulting Resident #2, resulting in physical injury and a contusion diagnosed at the Emergency Room.
