Lincoln Villa, a 740‑type residential care facility in Fremont, California, has been the focus of several unannounced pre‑licensing inspections under a change‑of‑ownership request. The facility, with a capacity of 80 and a current census of 68–69, is overseen by Administrator Lulin Wu, with Wendy Wong and Olive Manalasta as licensee/applicants and Divina Fernandez as director. Licensing Program Analysts (LPAs) P. Manalo and Y. Flores‑Larios repeatedly visited the site to evaluate compliance with state licensing standards.
The March 20 inspection revealed multiple serious deficiencies that made the facility ineligible for licensure. Unlocked medication carts were found in rooms 3, 15, and 31; the freezer lacked an internal thermostat; food was not stored in appropriate containers; emergency food supplies were inadequate; window screens had holes; staff lacked emergency flashlights; and resident records were incomplete. The LPAs documented these issues and directed the facility to correct them and fax a plan of correction to the California Department of Social Services (CCLD) by April 10, 2025. Fire safety equipment, including smoke detectors, carbon monoxide detectors, and fire extinguishers, was functional, but the presence of unlocked medication and inadequate record‑keeping posed direct risks to resident safety.
A second pre‑licensing visit on May 7 repeated many of the same findings and added a requirement for a construction permit following the fire department’s assessment of recent interior and exterior work. The LPAs again noted unlocked medication drawers, improper cleaning product storage, and inadequate emergency preparedness, and instructed the facility to submit corrections by May 16, 2025. The fire department also identified the need for a formal permit for all structural changes, a matter that must be resolved before licensure can proceed.
On May 30, LPAs P. Manalo and L. Fontanilla conducted a Component III case‑management review, providing a PowerPoint presentation and contact information to the licensee/applicants. No new deficiencies were recorded during this visit; the focus was educational and procedural. The repeated visits underscore persistent compliance gaps, particularly around medication security, recordkeeping, and emergency preparedness, while also highlighting the facility’s readiness for further corrective action before licensure is granted.
