Inspection Report Summary for Luxor at Mills Pond Rehabilitation & Nursing Center

During the reporting period, the facility received a total of 17 citations, comprising 7 standard health citations and 10 life safety code citations. These citations were identified in two on‑site inspections; the remaining eight inspections conducted during the same period resulted in no citations. The citation totals reflect the overall regulatory findings for the facility, with no citations arising directly from complaints.

The inspection conducted on January 14 2025 produced the highest number of cited items, with nine citations in total. Deficiencies identified under standard health included food procurement, storage, preparation, and serving practices; infection prevention and control; and the nutritive value, appearance, palatability, and temperature of food. Life safety deficiencies cited during this inspection encompassed building construction type and height; emergency lighting; hazardous area enclosure; maintenance, inspection, and testing of doors; sprinkler system maintenance and testing; and subdivision of building spaces with smoke barriers. The July 13 2023 inspection identified eight citations, with deficiencies in comprehensive care planning, food procurement, infection control, resident rights, electrical systems, fire alarm installation and maintenance, and smoke barrier subdivision.

Complaint data for the period from August 1 2021 to July 31 2025 show that the facility received 124 complaints, equating to 52.7 complaints per 100 occupied beds. Forty‑two percent of these complaints represented facility‑reported incidents. The Department of Health conducted 33 complaint‑related on‑site inspections at the facility, but no citations were issued as a result of these inspections. Consequently, the facility recorded zero complaints‑related citations and zero citations per 100 occupied beds. Across all categories—administration, dietary services, other services, physical environment, quality of care, and resident rights—no complaints‑related deficiencies were cited, and the total complaints‑related citations remained zero.
