MEADOWLARK GARDENS I, a single‑story group home licensed for six non‑ambulatory residents and a hospice waiver for four, has consistently demonstrated compliance with California Department of Social Services Community Care Licensing (CCLD) regulations. In annual inspections conducted in July 2021, June 2022, and September 2024, the facility’s physical plant, staffing, food service, and emergency preparedness were found to be in good repair and fully compliant. Records of resident care, personnel training, and medication management were complete, and COVID‑19 protocols—including a visitation station, hand sanitizer, temperature checks, and signage—were in place. Fire safety equipment, including charged extinguishers and operational detectors, met state requirements, and the administrator’s certification remained valid.

During the July 2021 visit, the licensing program analyst (LPA) Shobhana Frank noted that the home maintained a comfortable temperature, sufficient lighting, and secure grab bars in bathrooms. The June 2022 inspection, also led by LPA Frank, confirmed adequate supplies of perishable and non‑perishable food, adequate linens, and proper storage of hazardous chemicals. In September 2024, LPA Michael Tea documented that all resident files were current, the quarterly fire drill had been conducted, and the first‑aid kit contained all required items. Each unannounced review concluded with no deficiencies cited, underscoring the facility’s adherence to regulatory standards and its proactive approach to resident safety and care.

The only documented shortfall occurred during a July 27 2023 case‑management visit by LPA Kimberly Lyman, conducted in conjunction with complaint #22‑AS‑20230718163630. The investigation revealed that MEADOWLARK GARDENS I failed to submit a copy of an eviction notice for Resident 1 to CCLD within the mandated five‑day window, a lapse that could pose a health and safety risk.
