Findings
The facility was found to have multiple deficiencies including failure to maintain accurate and current service plans, inadequate monitoring and documentation of changes in resident conditions, incomplete RN assessments for significant changes, medication order errors, incomplete self-administration evaluations, inaccurate acuity-based staffing tool data, and insufficient fire and life safety training and documentation.

C0260 - Service Plan: General: Failed to ensure service plans reflected residents' current care needs, were readily available to staff, and provided clear directions regarding delivery of services for 5 of 5 sampled residents.
C0270 - Change of Condition and Monitoring: Failed to update service plans following significant changes and ensure resident-specific actions were documented and communicated for 3 of 5 sampled residents.
C0280 - Resident Health Services: Failed to ensure RN assessment was completed for 1 of 3 sampled residents who experienced significant change of condition.
