Summary of surveys and deficiencies for Merrill Gardens at Hillsboro Assisted Living Facility
Findings
The facility failed to ensure service plans were reflective of residents' current care needs, timely updated following changes of condition, and readily available to staff. Deficiencies included failure to complete RN assessments for significant changes, failure to carry out medication and treatment orders as prescribed, inadequate evaluation of residents' ability to self-administer medications, and failure to accurately capture and update acuity-based staffing tool data. Fire and life safety training and documentation were also deficient.
