In the unannounced annual inspection conducted on January 19 2022, Licensing Program Analyst Stephanie Williams found Mission of Love II to be fully compliant with Title 22, Division 6 regulations. The facility had no active COVID‑19 cases and maintained robust infection‑control protocols, including PPE, hand‑hygiene supplies, a designated infection‑control lead, and a documented testing and isolation plan. Operational requirements—utilities, emergency exits, food service, medication storage—were in order, and no health or safety risks were observed. The administrator, Jessica V. Villanueva, received a copy of the report and no deficiencies were cited.

During a required comprehensive inspection on December 19 2023, Mary Rico noted that while the physical plant, staffing, and record‑keeping were adequate, the facility failed to provide several essential plans. In a case‑management inspection on March 4 2024, LPAs Anna Bueno and Bianca Wolcott investigated complaint 56‑AS‑20240228135949. The inspection revealed that Resident 1 had been restrained to the bed with a sheet tied loosely to a rail to prevent falls, a practice deemed a health and safety risk. The sheet was removed immediately, and no physical injury was noted. A deficiency citation was issued under LIC 809‑D. The facility was informed of the citation and its appeal rights. This incident underscores the need for clear, safe restraint protocols and proper documentation of residents’ care plans.
