Visit Reason
The Minnesota Department of Health conducted an initial survey to assess compliance with state licensing statutes for an assisted living facility.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including food service violations, physical environment issues, fire safety and evacuation plan deficiencies, failure to offer designated representative opportunity, incomplete staff orientation, incomplete service plan, and expired medication not disposed.
