The California Department of Social Services Community Care Licensing Division recorded a series of inspections for Camlu Assisted Living, a Residential Care Facility for the Elderly (RCFE) in Stockton with a licensed capacity of 160 residents. The earliest documented visit on April 26 2021 was a Component II compliance call in which Administrator Alma Whitted confirmed understanding of Title 22 requirements and agreed to submit the required LIC 809 form. This call was followed by an unannounced pre‑licensing visit on June 23 2021, during which the licensing analyst Ashley Boothe noted that the central air‑conditioning system was not operational and that portable units were being used in all resident rooms. The analyst required a repair or replacement of the central system before the facility could be approved for licensure, and noted that the administrator had submitted building permits and was awaiting contractor bids.

A duplicate report dated June 24 2021 was later issued in error, indicating no legal or non‑compliance visit had taken place. The most comprehensive inspection occurred on October 15 2021, again conducted by Boothe, and documented numerous deficiencies. In addition to the unresolved air‑conditioning issue, the facility lacked working carbon‑monoxide detectors, functional window screens, and adequate ceiling tiles and drywall in several areas. Hazards such as exposed wiring, unsecured storage of paint and construction supplies, and loose bricks and a fallen tree branch were observed on the property. Staff communication was impaired because one employee did not have a functioning phone, and a call system that had been activated during a pull‑cord test failed to respond promptly. Resident documentation was incomplete, with missing physician orders for bedrails and unclear markings for oxygen‑use rooms. The analyst required a full remediation plan covering mechanical systems, hazard removal, staff training, and documentation updates before approval could be granted.

A follow‑up pre‑licensing visit on December 16 2021 indicated improvements in several key areas. Dining and common spaces were furnished and maintained in compliance, and COVID‑19 precautions, including isolation rooms, signage, and PPE storage, were in place. Emergency exits were unobstructed, and fire extinguishers and pull‑cords appeared operational. However, the report did not address earlier concerns about the air‑conditioning system, CO detectors, or the extensive construction hazards noted in the October visit. Across the four visits, a pattern emerges of repeated infrastructure and safety deficiencies, interspersed with efforts to correct them. The facility has yet to achieve full licensure compliance, as the October deficiencies remain unremedied, but the December visit suggests progress toward meeting regulatory standards.
